  • Birth Certificate issued by Registrar of Birth and Deaths or 10th Class Board Certificate showing date of birth.
  • Education Certificate with marksheet including Board Certificate in original.
  • Domicile Certificate with photograph issued by Tehsildar/District Magistrate.
  • 20 copies of good quality passport-size photographs.
  • PAN Card and AADHAAR Card.
  • Caste/Community Certificate affixed with photograph of candidate issued by Tehsildar/Sub-Divisional Magistrate/District Magistrate.
  • Religion Certificate issued by Tehsildar/SDM (if religion as “SIKH/HINDU/MUSLIM/CHRISTIAN” not mentioned in caste certificate).
  • Character Certificate issued by Village Sarpanch/Municipal Corporation SHO/Head of institution within last 6 months.

Relationship Certificate


  • For Son/Daughter of ESM/Serviceman/War Widow/Widow of ESM/Daughter of War Widow (adoption during lifetime of soldier).
  • One Son/Daughter only.
  • Widows of Defence personnel who died in harness must also produce Relationship Certificate.
  • Must be signed by Record Office and duly countersigned by respective ESM Welfare Secretary.
Affidavit on ₹10 Non-Judicial Stamp Paper prepared by ESM duly signed by 1st Class Magistrate mentioning relationship details.

Sports Certificate


  • Issued by State Sports Authority or Government body of recognized sports.
  • Must mention participation in International/Senior/Junior National/Khelo India Games/State level (as per team or individual).
  • Must be countersigned by 1st Class Magistrate.
  • Original Discharge Book (for ESM candidates).
